What is the capability of Allscripts EEHR to recieve and respond to requests for CCD from other systems/vendors. Below is an example of the statement made by another vendor and Allscripts response. Do you have any additional info and/or details that would help clarify the subject?

Statement from another vendor: We understand that Allscripts cannot respond to a query request from an outside system for a CCD. They can push out a CCD but not respond to a query from another system.

Allscripts response: Historically, EEHR has supported an inbound query/outbound response for a CCD via our UAI technology. This is in place at clients such as Sharp Healthcare to integrate their Cerner Millennium system with EEHR. This solution is an optional module of our product today and has been in place for quite some time. It requires the system producing the query to comply with our UAI protocols.

Our go-forward strategy is to extend our standards based CCD delivery system to support query in addition to publishing CCD’s. Allscripts is currently exploring the possible use of XCA to this end. Our products already support XDS.b/XDR exchange.

Additional response from Allscripts:

XCA is a gateway protocol from one HIE to another HIE. The definition can be found here: http://www.ihe.net/Technical_Framework/ ... -08-10.pdf

Epic and Cerner have both selected to implement this profile on their acute EHR's. Allscripts can query these implementations to request information about a patient within the Cerner or Epic system.

Allscripts did not implement this HIE Gateway profile on their ambulatory EHR's, because we supported both the XDS.b and XDR profiles to meet this need. Both of these profiles are intended to be used by individual EHR's to enable interoperability. Based on the fact that both Epic and Cerner have implemented the XCA profile, we've added this to our development roadmap, and expect to be able to support it in the coming year. Note that this profile may not be the best for a community of EHR's, as it's intended to be an HIE to HIE profile. That is, it works well with a single ambulatory EHR talking to a single acute EHR, but becomes less effective as the number of EHR's in the community increase.

We also support a web services layer on our EHR's that allows a user to query our EHR's for a CCD chart summary. This protocol is defined in our UAI specification, which is housed on a web site that is made available when an interested party executes an agreement to use UAI in the development of derivative software. This approach is being used by customers to query a patient chart summary from inside Cerner, and present it inline in their workflow using Cerner mPages and Allscripts UAI.
